The narrator is aware that depression, despite the Romanticism it engenders in the artist, is usually produced under circumstances of great boredom and pointlessness, like a housewife waiting for the milk van. One also gets the sense she's given up on life outside of this small lot of despair, because it is somehow more comforting, more real than the big bad world outside that could swallow her or any of us whole at any moment. So the mouth of darkness swallowed herself instead, but left behind her tongue...
